An incident​ that resulted in a tragic⁢ mid-air collision over Melbourne’s Port Phillip Bay was not properly discussed⁤ before the flight took off, according to a recent ⁤investigation.

The fatal accident occurred when veteran pilot Stephen Gale and skilled cameraman James Rose lost their lives after their aircraft collided with another ⁤military-style jet near Mount Martha, approximately 50 kilometers⁢ southeast of Melbourne, on⁤ November 19.

The Australian Transport⁣ Safety Bureau (ATSB) has now published the⁢ final report of the crash after ⁤an intensive eight-month inquiry.

The report disclosed⁢ that the flight manoeuvre that ​led to the​ collision, involving the lead aircraft Viper ‍1 rolling inverted while ‍the second jet, Viper 2, passed beneath it, was not specifically briefed ‍before the flight. Furthermore, the crew of Viper 1 ⁣was not informed ‍of this manoeuvre beforehand.

Following an initial⁤ attempt⁣ at the manoeuvre, the pilots of both aircraft discussed repeating it over the radio, which did not allow ‍them sufficient time ‍to evaluate the associated risks properly before trying it a second time.

Subsequently, during the second attempt, the right ‍wing of ‌Viper⁣ 2⁤ made contact with the right wing of the ⁢inverted Viper 1, resulting in ⁤a loss⁢ of control and substantial damage⁣ to Viper 2, eventually leading to ⁤its crash into the water.

“Proper pre-briefing of in-flight manoeuvres is essential for⁢ safe formation flying to minimize pilot workload ⁢and distractions,” emphasized Chief Commissioner Angus Mitchell.

Tragically, ⁢both James Rose and ⁤Stephen Gale lost⁣ their lives ⁣in‍ the crash, with their bodies later recovered ⁢from the wreckage near ‌Mornington.

The second aircraft, carrying two other individuals, managed to land safely at Essendon ⁢Airport.

The fatal incident occurred during⁢ the filming of a promotional video for Jetworks Aviation.

David Rose, father of⁢ James, raised concerns about the ⁢circumstances surrounding the flight, questioning the⁣ accountability of those responsible for overseeing the project and ‍the​ overall air⁤ safety system in place during such ⁢high-risk ⁤aviation activities.

“The​ loss of our beloved James is irreparable. ⁣We call​ upon the Coroner and CASA to⁣ delve deeper into ‌this​ matter,” he stated.

The investigation also revealed that the aircraft operator lacked the necessary certification for⁤ conducting the filming ​flight. Additionally, ⁣the surviving‌ pilot failed to adhere to medical restrictions by not⁤ flying only in aircraft with side-by-side ⁤seating and with a type-qualified safety pilot.
